QOF Year End 2025/26 Year End Preparation Checklist Available

The Calculating Quality Reporting Service (CQRS) has produced a Quality and Outcomes Framework (QOF) 2025/26 Year End Checklist to help GP practices with key actions in preparation for the year end.

QOF information is used by the CQRS to calculate GP achievement payment values for the current financial year, and to set aspiration payment values for the following year.

QOF reporting collections run throughout the financial year, with the information made available in CQRS afterwards. This allows practices to check it against the information in their clinical systems throughout the year and address potential issues in advance of the year end period.

QOF Checklist

The QOF checklist recommends the following actions:

1. Get set up on CQRS
2. Ensure you are participating in QOF 2025/26 to receive achievement payments (by 30 March 2026)
3. Agree to participate in QOF 2026/27 to receive aspiration payments (by 30 March 2026)
4. Report any Contractor Registered Population (CRP) changes for your practice to your Commissioner (by 31 March 2026)
5. Submit manual indicators for QOF 2025/26 (by 1 April 2026)

Expected QOF 2025/26 Calculation Timetable

• Participation will be extracted from CQRS on 31 March 2026.
• The GP System Data Extractions window is 1 April to 2 April 2026.
• Prevalence calculation and checks will take place on 5 April and 7 April 2026.
• Achievement and aspiration calculations will take place 7 April to 8 April 2026.
• Achievement data should be available to view and declare in CQRS on 9 April 2026.
