MMRV vaccine update for 2025/26 QOF calculations

An update from NHS England for commissioners managing the Quality and Outcomes Framework (QOF) 25/26 service.

On 31 October 2025, UK Health Security Agency and NHS England announced that from 1 January 2026 eligible children would be offered a combined measles, mumps, rubella and varicella (MMRV) vaccine instead of measles, mumps and rubella vaccine (MMR) as part of the childhood routine 2-dose vaccination schedule. The bipartite letter confirms eligible cohorts.

For QOF purposes, either MMR or MMRV counts towards achievement equally. However, it was not possible to reflect this change in the business rules for the VI002 indicator for 2025/26. This means that MMRV vaccines delivered between 1 January and 31 March 2026 will not be automatically counted in the VI002 end of year achievement figures in CQRS. Where a practice has delivered eligible MMRV vaccines in this period they should provide evidence to their commissioner as part of the annual declaration process in order that the numerator can be updated. The business rules for 2026/27 will be updated to include both vaccine types.
